Uncommon presentation of perforated Meckel’s diverticulum in preterm newborn

چکیده
A male neonate with gestational age of 30 weeks, weighting 940 g at birth, with respiratory failure right after birth, and radiological signs compatible with hyaline membrane disease. At his tenth day of life, the patient presented vomiting and abdominal distention, presenting with radiological signs of pneumoperitoneum (Figure 1). Initially, the neonate was submitted to peritoneal drainage, due to the lack of surgical conditions, and at the 19th day, after gaining weight and present with hemodynamically stable conditions, was submitted to exploratory laparotomy. During the surgery, a Meckel’s diverticulum (MD) was found, with jejunal perforation, hepatic blockage and obstruction distal to the blockage due to the development of adherence. Resection of about 6 cm of the jejunal loop including the perforated area was performed, with later termino-terminal anastomosis. The anatomopathological result was subacute diverticulitis with ulcer and severe peridiverticulitis. The neonate presented a favorable evolution and was discharged at his 82nd day of life. Meckel’s diverticulum represents the most common congenital malformation of the digestive tube, and is asymptomatic in most cases. Symptomatic cases of MD are rarely found, affecting less than 20% of all pediatric cases. Perforated Meckel's Diverticulum causing Intussusception in a Neonate
Intussusception is well-recognised in young children; however this condition is very rare in newborns [1]. Neonatal intussusception is usually caused by several conditions including Meckel’s diverticulum, intestinal polyps and intestinal duplication [1]. Meckel’s diverticulum can present at any age group; however rarely, a symptomatic Meckel’s diverticulum may present in neonates [2]. We report...
متن کاملPneumoperitoneum in a Micropremie: Not Always NEC
Pneumoperitoneum in the newborn is an acute surgical emergency requiring immediate surgical intervention to ensure survival. It refers to radiological evidence of rupture of an air-containing viscus with resultant soiling of the peritoneal cavity. A female baby was born preterm at 29 weeks with birth weight of 650 grams. She developed abdominal distension on day 6, and abdominal radiography rev...
متن کاملPostoperative intestinal obstruction caused by Meckel’s diverticulitis after appendectomy
Surgical treatment of the complications associated with Meckel’s diverticulum is widely accepted, but surgical management of an asymptomatic diverticulum found incidentally during laparotomy for other reasons is controversial. Appendectomy is the most common reason for laparotomy in many pediatric surgery clinics. The most common causes of an intestinal obstruction in the early period after app...
متن کاملIntestinal duplication with ectopic gastric mucosa diagnosed by Meckel's diverticulum scan
Gastrointestinal duplications are rare congenital malformations and they may vary greatly in presentation, size, location and symptoms. The presence of ectopic gastric mucosa (EGM) in them is not common and they are difficult to diagnose prior to surgery. We present the case of a child with long intestinal duplication with ectopic gastric mucosa who presented with just severe anaemia and delaye...
متن کاملMeckel’s Diverticulum Perforation: Report of a Case and Review of the Literature
Meckel’s diverticulum is the most common congenital anomaly of the small bowel. The majority of patients with this anomaly will remain asymptomatic; however, several complications can occur, including diverticulitis, haemorrhage, obstruction, intussusception, and perforation. A 39-year-old man was admitted to the emergency department after complaining of abdominal pain for the past few days. Up...
